Information and Referral / Admin Assistant
From $29,419 a year
Will work as part of a team. This position is dedicated to providing outstanding customer service, information and assistance. Primary responsibility is to answer calls to the agency and respond to walk-in consumers - providing prompt, courteous service to seniors, caregivers and service providers. Bachelor's degree is preferred, but would consider high school or Associates Degree with equivalent experience. Three years customer service experience and experience with public benefits helpful. Spanish speaking a plus.

Service Navigator
Job Type: Full-time
Connections Area Agency on Aging is looking for a caring and dedicated individual to join our team as a Service Navigator. Connections’ Service Navigators work with older adults to develop a personalized plans of care to address short-term and long-term concerns. This position requires a person who’s comfortable working with the elderly and or disabled population, and their care team.
As part of the Consumer Services team, you can expect to
- Provide person-centered counseling to help develop a person-centered plan to meet short-term and long-term goals.
- Arrange for or help make referrals to selected providers to initiate services needed.
- Coordinate and monitor the formal and informal service delivery to ensure that the services specified in the plan are being delivered.
- Collaborate and be a resource for case managers, physicians, law enforcement, county attorneys, DHS, and domestic violence agencies.
- Work with providers and the public to help facilitate the development of the Connections Plus program for consumers wanting to access private pay options for services.
Required Education: Bachelor’s degree in human service field; or currently licensed Registered Nurse. Can substitute up to two years of full-time work experience involving direct contact with people in overcoming social, economic, psychological or health problems in a human service field, for two years of the educational requirements Experience: Experience in working with the elderly and disabled and knowledge of community resources is a strong plus. Must have experience working with the public.
This is a full-time position with benefits and will work from either our Council Bluffs, Creston or Sioux City Office.

Transition Coach
Job Type: Full-time
Connections Area Agency on Aging is looking for a caring and dedicated individual to join our team as a Care Transition Coach. This person will work to ensure safe and effective transfers in the movement of patients across the care continuum, serving as the bridge between the professional staff in the hospital and nursing home setting and the patient and/or family.
Our Care Coaches act as patient educator-advocates and patient empowerment facilitator. Providing information and guidance to the patient and/or family for an effective care transition, improved self-management skills and enhanced patient-doctor communication.
This position requires a person who’s comfortable working with the elderly and or disabled population, and their care team.
Education: Bachelor’s degree in human service field; or currently licensed Registered Nurse.
